My son recently purchased a 1991 Toyota 4x4 pickup with a 22RE engine and 5 speed manual transmission with 188,000 miles on it. The first tank of gas got 20 mpg. Shouldn't it get better than that or is that typical? We'll check it again. Thanks.

A lot of things affect fuel economy.
Maintenance - When was the last tune-up? Air filter? Oil grade? Tire pressure? Is the air intake clean? The butterfly valve clean? Valve adjustment?
Is the braking system functioning properly?
Driving habits and conditions.
Tire size. The carrying of cargo. Accessories.

22RE is a four cylinder motor and gets better fuel economy to start with.
Fuel economy can be related to what I stated above.
Driving habits, maintenace, load

Because the 3.slow is a dog.
I get 20 mpg on my 22R (carbed) with 36" tires.

Make sure the front axles aren't locked in, that will create some extra drag.
My 94 4x4 with the 22re and 5spd, regular cab usually got 25 mpg.
My 91 2 wheel drive extended cab with 22re and 5spd has been getting 32mpg lately with over 200k.
You might want to check the timing chain, my timing would jump around some until I replaced the timing set.. and at the mileage you have, if the timing set hasn't been replaced the chain might be wearing through the timing cover close to a passage for the water pump. Antifreeze and oil would be a bad thing
