transmission hard shifting

i there,i have a gmc sierra 1999 2500 6.5 td,and sometimes the transmission his shifting hard,(one time a week)when shes ok she shifting normal,but she can do that anytime.do someone have any idea?I have put the scan on it,and nothing came out.
