Vent Control

Can anyone help me out with a problem I am having. The case that the vent control actuator is connected to in my 1997 Yukon is broken. I can't find the part number and don't know what the exact name of the part is. The plastic mounting that the actuator is bolted to is broken. Because of the loose connection to the actuator the vent control doors never work right. I would like to replace this so the system works as designed. Thanks for the help.
