97 venture slips out of od

I have a 97 venture with a 3.4 L engine, while driving on the highway at 60 miles an hour it will sometimes slip out of fourth gear and the engine will race until I drop down to 3. I leave it like that for a while and then put it back iinto D and it seems ok. Had anyone else had this problem? Any suggustions?
