93 ford ranger

i have a 93 4.0 ford ranger and the tranny only slips when the motor is cold, but when you put it in reverse regardless if the motor is cold of warmed up it will clunck or ingages very hard. does anyone know what the problem could be??? just a simple flush new filter and new fluid?? please reply back thank you
