this an automatic or manual?
If its an auto check the trans fluid level and if its dark and smelling funny. Could need a fluid and filter change.
But if its a manual, check the fluid level on the clutch.
This could also indicate a faulty/dirty IAC, or a vacum leak on the intake, or some thing to do with the fuel system. Like a fuel filter cloged or the fuel pumps weak(check fuel pressure).
Hope this narrows it down, lets us know! And Welcome to Automotive Forums!

Since it is acting up so much on the hills, I would suspect that it would be a fuel filter. It sounds like the classic symptom for a clogging fuel filter. I would start there, but I would also check the other items listed by Xeroinfinity. The replacement filter is not too expensive, if you are going to do the job yourself you will most likely need a special tool to release the filter from the fuel lines. If you decide to do the job yourself be very careful it is a messy job and you will probably get gas on yourself.
Even if it turns out that the fuel filter is not the main issue, with 189,000 miles it would be a smart thing to so simply because of the number of times you have had to put gas in the tank.
I am recomending this on the assumption that the fuel filter could be the original filter.
Hope this helps,
