CD DECK good dash lights went retarded

ok so we wired the deck up. i was wondering.

we installed the deck. i matched the colors up. but then her dash lights werent working.

so i put the ground to the metal o ring thing that the stock deck had for its ground. then the dash lights worked. but it says the headlights are on even when we shut them off. (says so on the dash) so what did i hook up wrong doing the deck? any ideas? im very experienced at car audio, but this is messing me up.
