bad o2 or pcv?

ok, ive had my car for like 3 years the gas milege was fine, now it suks a quarter of my tank to go 30 miles. my tires are balenced and have the right psi. bad o2 sensor? bad pcv valve?

Yeah. Replace the fuel filter too.
