bushing,transmission or engine?

After asking several questions, I am getting more confused. maybe I should combine all the sympons as better clue.

Sympton1:
While it is clod (30 or below) outdie, the 1999 T&C AWD (@105K) takes about 10 or more minutes to "warm up". Here warm up I meant temperature gauge go up to middle. Without reaching that position, the van feel powerless, vibrates especially at slower speed. Some suggested maybe throttle needs cleaned but my argument is--clogged throttle should have problem no matter it is cold or warm...Am I right??
Is it related to transmission? I guess not. As far as my knowledge goes, tranny always accompanies with drifting, slippage or bang shift. That is not my case at all.
The powerless/vibration feel is also especially obvious when the wheel is turned more than 45 degrees. Very gently applying acclerator or floor it is less vibrating as medum force on acclerator. Some suggested engine mount. I am questioning why engine mount vibration onlt occurred at the medium force?

Sympton2,
Bushings at sway bar were all replaced and checked in this year. The van rattles/clunks while passing speed bump at slow speed. It seems less noisy passing it at higher speed. If it is anything related to suspension why it is less noticable at higher speed?

Sympton3,
The van seems hating driving between 20 to 40mph most, once it reaches 60 or higher, feels new again. Of course, it is no longer new, driving up on slope shows its age but I love the performance on highway still. I had it as my main highway travel car up to two years ago until I got a 2004 Sequoia. It has been my wife's stop and go commuting car in the past two years. is it part of the reason it shows age fast since sitting on bench all the time? Oil & filter change is consistent on this vehicle.

I hate to dump this van but it is creating excuses for my wife this year...Someone please help me to save big $ on a new car!

Thanks

Should I summarize the whole thing as--the power does not transfer to axle to move the tire forward, a loose joint or something?
All these sympton seem to conclude to this but I can not explain why floor the accerator will end this problem faster without too much vibration?
