96 altima fuse diagram

I am missing the diagram to my girlfriends 96 altima GXE. the one right next to the steering column (I know its on the back of the fusebox cover, and she doesnt have it). If anyone can snap a good pic for me it would be greatly appreciated. I am also having problems with the blower motor, it comes on and goes off (and stays off for long periods of time) I looked at most of the wiring and it looks to be intact and as far as i can tell i have no open fuses is this on a relay? or is the motor prob just shot?
