No first gear

I have a 96 S10 6 Cylinder 4X4 automatic trans.
When the engine is cold, everything works fine. But drive 5 miles and I lose 1st gear. If you go from Neutral to drive, you can hear/feel two thunks, first gear is thunk one and then right away there is the second gear thunk.
Fluid change and flush did not fix problem.
Where do I start looking?
