Clutch Problems

My clutch in my 95 GSX is basically gone. Everytime i step on the gas while driving my RPMs will go sky high but my car wont go anywhere until about 5 secs later, then it grabs and takes off. I obviously need a new clutch, so does anyone have any suggestions on which type. And also some1 told me i should send my tranny out and get new parts in it as well as get the new cluch. Should i do that, or is it not worth it considering my tranny is fine. Thanks
