Front Right Brake Grinding/Clunking

I'm pretty sure I finally figured out this problem. I'm getting a bad squeak, grind/clunk everytime the brake is slightly applied, and it will disappear if I push harder. After tearing everything apart, it appeared that my Slider Bolts were rusty. I grinded them down and lubed the sleeves up, but it came back a couple of weeks later. I'm going to replace the bolts completely today, I'll let you know if it fixes it.

I never did replace the bolts, because the bolts were still fine. I did notice, however, that the rotor and the caliper were not square with each other. This, I suspect was the problem. If anyone has this problem, check there first. It's been almost a week, and still no clunking, so I'll let you all know if it comes back.

Yeah, that noise is back again. Any other suggestions to fix this?

How about the bolts that hold the caliper bracket to the knuckle?

I checked them and they seemed like they were almost stripped, like they were forced. I took them out and put some anti-seize on them and made sure they were in correctly. It looks like the pads will move away from the rotor - like, they slide, I guess. Nonetheless, they don't sit sqare with the rotor. This happens about a week after I readjust them. Should I try re-threading those holes and getting new bolts?
