Tranny Problem 1986 f250

i have a 1986 F 250. i've been having problems getting it to shift. It also grinds when i put it into reverse. i'm not sure if there is a way to fix this problem or if i'm doing something wrong when i drive it. Any help or advice will be greatly apprecitated.
