power seat

I have a 1999 Lincoln Continental. The driver's power seat is having problems. Instead of moving smoothly forwards or backwards it's jerky and sometimes gets hung up and just makes a noise. Does anyone know what the problem is? If it needs a new motor, do you know where to get one?
