Oxygen sensor problems

I have a code 157 low voltage on one oxygen sensor. Does anyone know what could cause this?

I also have a code 137 and 138 on the other sensor which is a Low voltage and high voltage problem. How can you get both codes at ounce or do you have to replace the sensor?

Can anyone help me out.
