PO401 EGR Failure

I have a 96 T&C with 3.8 and have a OBDII code of PO401. I know that it can be one of the following:
The DPF (differential pressure) sensor is faulty and needs to be replaced
There is a blockage in the EGR (most likely carbon buildup)
The EGR valve is faulty
The EGR valve may not be opening due to a lack of vaccuum
What is the most common? Any suggestions? Where is the DPF Sensor? The EGR Valve?
