93 head rebuild?

My 93 3 cyl runs very rough when you first start it after sitting a few days, and has smoke out the exaust. The smoke goes away after a few seconds and the engine evens out. It does have a rough idle normaly, but especialy after sitting. Could this be something like valve stem seals? Thanks
