First Gear Pops Out

I'm not sure if this is normal. Others I spoke to told me it happened to them, but went away after a while. It hasn't gone away in my case.
I have a 1998 Toyota Tacoma v6 4x4 Xtra Cab 5 speed. Sometimes when I am in first gear, as I acclerate, the gear pops out of gear. If my hand isn't there, it makes a loud sound. I've tried to fix it by replacing the bushing and the rubber washer (the one that sits underneath the stick shift). It stopped popping out for about a week, but then continued to do this.
Anyone familiar with this problem?

Worn shifter forks or bent shifter rails are probably causing it to pop out.
