Could be the timing belt tensioner. Makes a loud ticking sound when it goes bad.

If it really is caused by the lifters, it is a common problem among rodeos and passports.
A dealership actually told me to add some transmission fluid about a week before getting the oil changed and that this would relube up the lifters sufficiently to reduce the ticking.
I don't know anything about engines and I haven't yet taken their advice about mixing those fluids, so be forewarned and do your own investigating before adding tranny fluid into your oil reservoir.

I have also heard that adding tranny fluid will help. But not a week in advance. I ran mine for a day with a quart of tranny fluid in it. when I changed it, it was deffinately blacker than usual, and now several changes later my oil still looks clean even after 3k miles. Honestly not much difference with the ticking though. Found Sea Foam did a better job. Still ticks on cold startup, but now its for only about 5 seconds then quiet as can be.

Check the tensioner. What is its history?
Also, if it's running rough, pull the plugs to see if a cylinder or 2 are missing. Could be a coil pack problem.

I would NOT be adding ATF to the crankcase, all this does is thin the oil out - that's it.
If you're already running a 5w-30, the ATF will thin the oil even more, i.e. can't be too good on an older engine in a hot climate.
If you want help to clean up your engine, although a bit slowly, run Rotella Synthetic 5w-40 diesel/gasoline oil and a Napa Gold filter. Trust me, this combination is something this engine loves and runs great on.
The Rotella synthetic is a heavy duty oil with higher additives/detergents, and happens to be an oil that a long time Isuzu mechanic specialist -JLMOND (IIRC the spelling) highly recommends for these engines. He's taken apart and rebuilt numerous Isuzu engines and recommends this oil as the best for the overall health.
This oil can be found at Walmart for about $16.?? for a 1-gallon jug, and 2 qts should readily be found as well.
You MUST use this oil with a good oil filter to get the true benefits - do NOT use Fram, Supertech, ACDelco on this engine, IMHO, they do not filter good enough.
