Need help with 93 Topaz trouble accelerating, and high idle

When my car is first started, it idles very high, I have to "goose" it, and it drops back down where it belongs. When I am driving it, it has started hesitating, and it is getting worse, it is very hard to get it to accelerate. Can anyone help me out with these problems?? Thanks!
