Rough Idle 5.4L Triton

What would cause rough Idle on 5.4l triton v8, but as soon as you put it in gear it smooths out?

Have you checked for vacuum leaks? The pcv elbow at the back of the throttle body is notorious for leaking.

Thanks for repling but I have no vacum leaks.
