sluggish 95 cutlass supreme

Ok....I paid a little more attention tonight when I drove to and from work on how it acts and what bells and whistles go off.....When I go to slow down it doesn't shift, once stopped it doesn't die, when I start moving again the RPM's flutter a little bit between 2000 and 2500, the car jerks and lunges and around 30 to 40 mph seems to work out the jerkies.
I just wanted to make sure I had the story right, I apologize if it sounds repetitious.
