Does it drive when on "OD" after it shifts up from 3rd, or does it just rev up?

Your OD band is toast.
Unfortuneately, it's time for a new trans. You can lock-out OD and drive it for quite a long time however.

By locking it out you mean keep the OD off by means of the switch???
The biggest thing I am losing is the gas mileage since it does have OD anymore Correct ?
Howver I should not complain the car has 255,000 miles the engine still runs real well and the cars does everything else very well.

Right, push the TCC switch after you start the car each time, and you will only hurt you fuel consumption rating.
Beware of "rebuilt" transmissions to. VERY VERY few places, even the national brands do even a decent job. If you decide to replace it, I suggest a Factory Ford reman.
