keyless entry programming from scratch

Can anyone tell me how to program a keyless entry for a 2000 Dodge Grand Caravan from scratch. We have lost our original remote. The manual tells you how to do it when you have an existing remote but it tells you nothing if you have to start from scratch.

You'll have to take it to the dealer unless you have one of the original remotes.
