clutch adjustment question

i have a 1998 integra Ls sedan wiht 90k miles, recently my clutch catch point has moved and occasionally it will slip in first gear (clutch is fine 80% of the time but it just happen today so it might get worse).. i was wondering if my clutch is shot or if i just need to get my clutch adjusted..
