'96 3.2 Flywheel/Flexplate Problems

Anyone ever have flywheel/flexplate issuse? My Trooper has 120,000 on it and it keeps spinning the centers out of the flexplate. The first time it happened the flexplate was replaced with a junkyard part, wich broke almost right away. So I replaced it with a brand new part from the dealer, 400 miles later that one broke.

The center is tearing right out in an almost perfect circle right where it bolts to the crankshaft. I'm baffled, I've never seen anything like it.
