When the trans shift gears my cv joint makes a clicking noise it does not do it when turning. I noticed when changing the oil the cv boot is pretty much gone. Do I need to replace the cv joint and bearings or just the boot. Any comments or suggestions is greatly appreciated. I'm not to familar with the drive trans or cv joints. THANKS

that happened to me. if you caught it early enough replacing the boot should be sufficient.

If you had caught the failed boot immediately, you might be okay, but with noise from the CV joint and a failed boot, the joint has probably suffered damage from the lost grease. Replacing just the boot and grease won't fix the wear.
I'd look at costs to replace the joint or the entire half-shaft. The labor is about the same as the half-shaft gets removed from the car either way.

P.S. the wheel bearings are probably fine as the boot doesn't really protect them. You may want to have them repacked with fresh grease while the other work is being done if the car has significant miles on it.
