I'm conducting an experiment for school, and part of it requires me to gather information. I've put together a short survey for people to take (short as in 5 yes or no questions). Nope, I wanted to keep it short so I can get as many people in as little time as possible, plus, I know I'm lazy and wouldn't really feel like answering a long winded question, and it provides all the information I need.
