Jumping out of gear

99 Corrolla Liftback (I am in Europe, might be different from N. America), manual transmission.
The transmission regularly jumps out of 4th gear. This used to happen oly when trundling along at constant low speed but now happens when accelerating as well. The dealer adjusted the linkage about a year (about 12000 miles) ago which did seem to improve matters but now it's got worse.
Any ideas?

Does it feel like there's allot of free play in the shifter? Does the shifter "travel" putting it into 4th feel the same as the rest of the gears?
You could check the linkage on the trans to see if the rubber bushings at the ends of the shift cables are worn.
If it's not in the linkage, it may be a bent shifter fork in the trans and/or the trans may need to be rebuilt or replaced.
