I also have a 2002 4Runner and just after I bought it (brand new), I started hearing a faint noise when the A/C clutch was engaged. I took it to the dealership to have it looked at and their troubleshooting process included the following:
1. Disconnected as much of the accessories and ran the engine to find out if the noise was coming from one of the accessories or the engine.
2. Checked the valve clearances. In order to do this, they had to remove the throttle body and air chamber.
3. Checked the main bearing clearances.
The conclusion they came up with was that the clearance on two of the main bearings were out of specifications. I asked that the Southeast Toyota's district manager get involved and he authorized the dealership to replace all the main bearings. After the repair was made, my 02 4Runner's engine never sounded the same (it sounds louder than before the repair and sounds like a much older engine), I hear ticking sounds when the engine is cold, and the idle is not smooth anymore (I feel vibration on the steering wheel and driver seat when at idle and gear engaged). I have complained about these things to the dealership but they told me that they could not find anything wrong and no trouble codes appeared when they ran a diagnostic check. So now, I'm just trying to live with it.
How about your 02 4Runner? Is everything still the same as it was before they replaced the main bearings?
