I have an 02 silverado with a 5.3 engine has 140k. My problem only occurs during cold weather starts. It struggles to stay on has a miss but only during initial start once it's warm it runs fine. Kicks out generic code bank 1 running to lean.

what are the DTC numbers.
also swap O2 sensors see if the DTC changes or not .
the ECT on the driverside of head next to cyl #1 is a possible cause to the poor cold start/run issue.
just to be safe measure the fuel pressure spec is 53-62 psi. should be 60 psi key on engine off. when the engine starts the pressure wil drop a few PSI. 55psi.
any pressure below 53psi is the fuel pump.
replace fuel filter every 30-40,ooomi keep fuel level above 1/4 tank at all times. if you did not do this the fuel pump will die.

ya thanx how come they just dont go out and only on cold starts is it cuz the demand for fuel when its cold is more demanding

with those pressures I am quite surprised it does run. with 50 psi most have severe engine running problems.
possible the pressure guage is off a little . that caN happen if its shocked/dropped.
use a delphi fuel pump. replace the fuel filter. since you have 140,oooplus on it replace the fuel pump relay and clean the fuel tank/fuel pump ground on the frame rail next to the fuel tank driverside..
do not use an airtex fuel pump . these are prone to failure after a few months.
