2000 2.3 Ranger Accelerates Rough

My 2000 ranger has a problem.
2.3L, Manual, 2WD, 150K
It does have a code but is unrelated, the code is low EGR flow, due to a bad DPFE. I have inspected the EGR valve and the intake hole for it they are clean as a whistle.
Also the problem started way before this low EGR flow crap. Maybe 2 years ago.
The problem is when I give it gas quickly in neutral the engine chokes up almost stalls and then revs like it should. If I give it gas slowly its OK. So when stopped at an up hill light, and you need to give it gas quickly so you don't roll back too far the engine chokes up, almost like a squeak noise.
Any time I give it gas to fast it almost stalls and chokes up. Also if I am in a low RPM for any given gear the car hesitates. I know this is normal but it hesitates to early to be normal I have to keep it in a little higher RPM's to keep it running smooth. Also the truck starts rough and has gone through 4 starters in 3 years. The bendex goes bad.
What could cause the engine to choke up when you first give it gas quickly?
The fuel pressure is at 75psi
I have changed the fuel pump 1 year ago.
Has injectors and fuel rail flushed.
Changed clutch 2 weeks ago
Checked cylinder pressures and they are good.
I run synthetic only.
