Need Clutch Help ASAP!

If anyone's got a lot of experience diagnosing clutches please give me a little expertise. I'm no rookie when it comes to diagnosing clutches, but need some help on this one. Money's tight & I'm working two jobs to make ends meet. Need to get this done right the first time & A.S.A.P.

-I have a 2001 Ranger 2.3 with 118,000 miles on it.
-I replaced the clutch and master cylinder upon failure at a suprising 78,000.
-4 Months ago my clutch started slipping ONLY after a half hour of stop & go.
-Since two months ago, the clutch has been operating normal under all conditions.
-Last weekend it failed completely and left me stranded & is my daily driver.

-I approached a turn as I was traveling 55mph.
I pressed the clutch and attempted to shift into 2nd gear early (at 55mph) because I had my girl in the middle seat.

-The safety feature did not allow me to shift into 2nd at 55 mph (normal) and it bit just a little bit.

-I made the turn, shifted into 2nd, and released the clutch

-2nd gear did not grab at all. So I shifted into 3rd & it rattled for a few seconds. Continued into 4th and it drove fine.

-Upon reaching a stop sign, I shifted into first and had nothing. I can shift into all gears including reverse without pressing the clutch. There is no friction at all on any gears.

-I am about to pull the transmission & found excessive amounts of clutch disc fiber coming out of the transmission casing so I know the disc will have to be replaced & flywheel resurfaced.

DOES ANYONE KNOW WHAT WOULD CAUSE THE CLUTCH NOT TO HAVE ANY EFFECT AT ALL?
