Check engine - coolant temp reading

Pontica, Grand Am Se, 1996, 104,000 miles. Had my check engine light come on, the code said it was the coolant temp sensor. Replaced that and a day later, the check engine light is on again. I was told to maybe replace the fan relay but can't seem to find it. Where is the fan relay located? Also, when I start the car now, it is fine until the fan kicks on...once the fan kicks on, the temp gauge goes to nothing, the check engine light comes on, the RPM's spike to 3,000 and the engine sputters momentarily. Any ideas?? Thanks.

If the fan kicks on , the fan relay is fine....:alien:
