Vacuum problems

I have a 1995 2.2 liter pontiac sunfire engine with a vacuum leak around the plug-ins on the vacuum router plate that sits atop my engine's throttle body. My question if someone can answer it, is what in the heck is this part called? I can't find the name of it on any website. Or, what are the rubber plug-in grommets called?, (4 of them two large ones and two small ones), that plug into the top of throttle body and are parts of the vacuum router plate. These worn grommets, or whatever they are called, are causing me severe engine problem headaches when accelerating from stoplights in stop and go traffic.
