not running smooth

I'm in the military, on a island in the middle of the Atlantic and my van isn't running like it should. i can't afford to have a bunch of stuff shipped out that I don't need.
Here's my problem- it's not running smooth. it "putt's" every so often about every 3-5 seconds. Also, i'm getting a grinding sorta sound from the rear of the van. Any thoughts? Thanks for the help.

my van did the same when the fuel pump went bad. listen at the tank to see if the noise is coming from there.

hi islander as far as running ruff i tuned it up changed antifreeze changed thermostat and replaced cooling temperature sensor which is located right by the thermostat neck
