Angry 3rd time water pump disc (where fan clutch bolts on) coming off

About 2 months ago my water pump malfunctioned and the disc at the end of the shaft where the fan clutch and pulley bolt on came off of the shaft. In doing this the fan sliced my radiator. I put a new water pump, radiator, fan blade and serpentine belt in and then it happened again 3 weeks later. Prior to the second time i had got into an accident and my fan shroud was broken which bent my fan a tiny bit so the insurance company paid for it. Thinking the bent fan caused it the second time, I replaced everything plus a new fan clutch and i drove it about 30 miles and it happened again. If you guys have any ideas what is causing this it would be a great help. I don't want to replace everything again just for it to happen again. Thanks

My first question would be are you putting a new water pump or a rebuilt pump . If they are rebuilt I would be getting a new not rebuilt pump. Next I would question if the fan clutch is fitting properly, maybe it is pulling the disc off if it doesn't fit right, sort of like a puller. Sorry not much help.

I am putting new water pumps in and not rebuilt ones, at least that is what NAPA is telling me. They are currently in the process of sending the pump in to the manufacturer to see if it is defective or not, but it is doubtful because is has happened twice in a row.

You can get a bad batch on parts.
Go dealer oem parts or another brand.

Sorry, I should have said go to dealer (oem) when I said new.
