Chev / GM Vehicle Recall

My dad got this in the mail yesterday and thought id post it up for all incase someone didnt get a recall notice if there vehicle is mentioned.

Personal information and VIN number have been blanked out for my privacy.

I apologize in advance for the low quality of the scan.
