Angry O2 sensor Which one is bank 1 sensor 3?

Malfunction on O2 sensor, bank 1 sensor 3. Anybody know WHich one is Bank 1 Sensor 3? I had it replaced about 8 months ago.Now Im gettin a code for it again. I need to do it myself to save the $. I can only find three of them and I was told there were 4. Any Ideas why its malfunctioning again?

on a 4.3L it would be the one between the catalytic and the muffler. What's the code?

I meant P0142, malfunction, it coded 2 times. Does it need replacing or is it just gettin tweaky?
