94 voyager transmission

I have a prob. with my Voyager trans, it shifts fine and works well except for when I slow down to stop or to make a turn it seems to
shift out of gear for a sec, and then suddenly drop into gear , 1st it
seems. I was wondering if anyone knows what causes it, I would like to
fix it before it ruins the transmission. thanks

The fluid level seems fine,it doesn't do it all the time it is maybe twice a day, while driving in town.

This happened to my 94 Grand Voyager as well. The fix was to replace the two speed sensors (input turbine and output) on the transmission.
