Unhappy 2004 gmc envoy xl (stalling?) (rough idle)

I have a 2004 GMV Envoy XL. 184K V6
When the car is in park with the hood up watching it. The fan will slow down and the motor will move around. Almost like it is about to die. And then a few seconds later the car sounds like someone hit the gas. And then everything goes back to good. Also when idiling the rpms do move some but not alot. It also has a loud repeated tick sound.
Recently I replaced the water pump but this problem was occuring before that.

Can't tell what the ticking is from here, but get or have the throttle body cleaned, may correct the stalling.
