2004 ford freestar minivan (tranny limp home mode)

about a month ago my wife called me and told me the car was really running high rpm's.

I took it out and sure enough it was no longer shifting properly and it had a transmission light on the dash. I shut the car off and restarted it the light went out but it was still stuck in limp home mode.

I brought it to a tranny shop assuming my next stop was the junkyard. during the drive test with his code reader on it he witnessed the problem. Now here is where it gets a little odd.

he cleared the code and we drove it about 5 miles at all legal speeds and the problem was gone. Now a month later the problem has still not showed up again.

So two questions?

What do you think could have caused it to go into limp home mode?

and

Since my code reader wont touch tranny codes how can I clear the problem if I am out driving and it happens again?
