99 escort ZX2 DOHC timing belt

I changed the timing belt and 2 pulleys plus the tensoner on my 99 escort zx2 2L about 2000 km ago. Now the belt has drifted and the belt is starting to shred. Any idea why the belt would drift out from the motor and shred

If the belt was replaced and then began shredding, the shim washer on the crank may have been installed between the lower crank gear and the block. The shim washer should be between the crank gear and the crank pulley.

I would suspect the tension on the belt was not set properly.

Thanks I suspect maybe I didn't tighten the tensoner enough/ I will review the setting before I loosen it off.
