Drive belt on 1988 Ford Tempo pop off

Drive belt on 1988 Ford Tempo slip off.
Not sure why.
Bracket obstructing view of tensioner seems to be attached to engine.
tensioner seems to be not in line with the ribbed pulleys.
Is this tensioner damaged ?
Not sure how it looked in the beginning.
What is the best way to check if this tensioner is still good and put the belt back on.
Also what keeps the tensioner in line with the pullys?

Do you have the 3 belt system or 1 belt system?

2 small belts , 1 large belt
The tensioner is right above a large pulley and the wheel on the tensioner is resting on the the larger pulley.
2 small belts , 1 large belt

It shouldn't go that far, the tensioner is bad.
