2001 E 450 7.3l has a dead spot

my 2001 E-450 7.3l turbo diesel has a dead spot at about 1/2 throttle where it blows a lot of smoke out the tail pipe and sputters until I give it more throttle and then I hear the turbo spool up.:banghead:
