The brake pads we use are Performance Friction Carbon Metallic. We have had really good luck with them. if you want to really enhance them, look at some cyrogenic rotors to mate them up with. They are a little pricey, but you can go through about 3 sets of pads and not need to resurface them.

Just some food for thought. I work for a municipality garage (City). When I took this position, the police intercepters were using drilled/slotted rotors from carquest with the ceramic blend pads that were suggested by the carquest employees. Apparently they have been using this combo for some time and not paying attention to the fact that the ceramic "dust" was cloggin up the slots and holes, therefore overheating the rotors and cracking them. I changed it up a bit. Same rotors, but with oreilly thermo quiet pads. I have yet to see a clogged and burned rotor in 5 months now. hhmmm...

Not susre of the number, but they have different numbers for different years (01-05 model years). I dont know what year you are looking for. If you want I can find out for you. They have the Carquest name all over the box. Severe Duty brake rotors, Drilled and slotted.
