grinding noise from clutch

my friend has a 94 f150 with the straight six with 200,000 miles. with the clutch pressed in there's a rattling/grinding noise. with the clutch engaged it sounds fine. the noise seemed to only happen either in neutral or with the clutch in while moving. any ideas on what it could be.

sounds like either a throw out bearing or pilot bearing. either way it requires dropping the tranny

I heard the same noise from my '82 f150. Someone told me it was the throwout bearing on it's way out, and sure enough the next week the clutch completely died. I am taking everything out this Sunday to replace it.
