I have a 1990 4runner with the v6 engine and 230,000 miles. For quite some time now i have had an issue when the engine is cold. When accelerating form a stop, the engine will hesitate and sometimes stall out. When it stalls it also is quite difficult to get restarted. The problem appears to occur every time the engine is started cold. But once the engine has been running for about 15 min or so and warmed up, it runs beutifully. I have pulled out the cold start injector and it apears to be fine. And I have also done some minor troubleshooting on the EGR system and have not found anything obvious. Can anyone point me in the right drection?

Have you checked the signal to the cold-start injector? The injector may work ok, but if the signal is absent when the engine is cold, then you will have the trouble you are describing.
