97 CV when temp goes up car runs BAD!

I've got a 97 CrownVic that runs GREAT when the temp outside is under 60 degrees, but once it gets to 75+ degrees outside the car idles really bad, shudders, jerks, and just feels like i'm driving something from a chocolate milk commercial.
OH Forgot to mention when its running like crap it also sounds like a diesel truck or a bucket of bolts
it only does this during the hot weather days, I can drive it 300+ miles on a cold winter day and it feels like sitting at home watching tv.
on a hot summer day we call it the Mobile Love Machine, just add a few quarters and it starts shaking.
we have changed out the spark plugs, wires, coil packs, thermostat, every filter under the sun, flushed the radiator, rebuilt the trans at 225k, it shows no codes. CEPT on hot days it will send different cylnder misfire codes. sometimes #4 or #5 (both sides of the engine will code a different times, its random almost)
on a summer day even at idle it will shudder and jerk, or when in reverse it feels like the rear axle is just gonna bounce off.
cold days nothing is wrong, shifts perfect, smooth and quiet.

ok so i just posted and fixed the problem for under $20 and less than a day.
I read a ton of posts that mentioned cleaning the MAF and also using SEA-FOAM (sucking it in via the PCV value), Plus I disconnected the neg cable from the battery and let the car sit for a good 10mins.
its kinda cold today and the car hasnt set off any codes yet either, knock on wood even if it codes I hope it runs as smooth as it is right now.
